Discussion Oral malodor was apparent problem among general population and evidences exposed that it forms about 85 of all bad breath 13 It affects peoples of all ages mostly during morning hours This study demonstrated halitosis among OPD patients of different age groups with different factors but most of our results have shown confinement with previous studies on halitosis and some new observations The current study focus on knowledge attitude and practice regarding halitosis In previous study 13 shows that out of 57 reporting bad breath 30 were adults 50 years of age and above in his study on halitosis and its relationship with mouth breathing observed among total individuals having mouth breathing and halitosis approximately 70 were 14 16 years of age children A similar study was also done by another researcher 14 who found 75 of total respondents having oral malodor and breathing via mouth to be children age 12 14 years

But in our study 54 7 individuals reported bad breath 32 9 individuals felt smell in your own breath 17 4 study participants had bad taste in their mouth and 6 1 individuals observed mouth breathing were aged between 17 to 65 years and the most likely explanation can be as mouth remains open majority of times decrease salivary flow buffering power of saliva and water evaporating in saliva among mouth breathers might be a contributing factor to halitosis When we grow old we obtain various diseases and weak immune system that s why older people often have systemic problems and diseases including sinus issues congestion that force them to breathe from their mouth and thus have problem in maintaining their oral hygiene resulting in halitosis 15 In this survey 65 3 of the individuals had responded that they had bleeding gums while brushing their teeth had bad breath 15 5 of the individual responded that their gums not bleeding during brushing and 19 2 of the individual said they had not notice about bleeding gums In comparison of this study 4

47 persons worried about their bad breath same as in 4 previous study 16 In this study there is no specific age show highest no in comparison of previous researches highest number was 30 observed in respondents age between 40 50 years old 17 In their study 66 participants have bad taste oral malodor similar study 18 have shown halitosis with direct correlation of the number of tongue coatings When respondents were asked regarding the history of medical problems 20 3 replied to have Gastrointestinal GI problems in comparison of past study 29 have GIT problem 13 9 have a history of the respiratory problem but in previous study 29 5 have respiratory problem 19 Among other factors inhabits 47 1 of respondent consumed raw Onion 18 2 use Ginger and 2 1 use garlic but in past researches, 35 use onion ginger 20 have a habit of the use of garlic 20 This study suggests that halitosis induced due to raw onion and garlic

The onion contain high concentrations of sulphur which can pass through the lining of intestine into the bloodstream and subsequently be released into the lungs and then exhaled so found direct correlation of onion with halitosis 21 22 and ingestion of garlic Allium Sativum is well known to cause bad breath so suggested that garlic breath odour comes from lungs not from particles retained in the mouth In our study 49 5 participants knowledge that brushing prevents gum diseases and 41 1 had no knowledge about this We also observed 30 use dental floss these findings can be suggestive due to the fact that flossing daily have a masking effect without really eliminating the real cause In the past many researchers have demonstrated effects of halitosis on social life The 4 47 participants worried about bad breath in this study but in past study individuals who worried about bad breath are 21 1 23 It was suggested that 20 5 had their social life affected as a result of this and 32 40 individual between age of 20 40 years old so this can be explained as younger population often socialize more than older people and so have more chances to perceive negative impact of halitosis during socialization 24

A considerable percentage of our participants 47 1 reported malodor on awakening and rest of them experienced when they were during day time or after meal in comparison of Libyan study on The prevalence of self reported halitosis and oral hygiene practices 69 found that bad breath was reported more on awakening Sleeping being hungry or thirsty would certainly reduce saliva flow and lead to oral malodor However in those conditions the bad breaths are temporary and disappear once food or drinks are taken and should not be regarded as true malodor Bad breath can have a distressing effect that may become a social handicap and the affected person may avoid socializing with people In our study tooth brushing mouthwashes and flossing has been implicated in oral malodor Our study proved that there was significant relation between brushing and experiencing bad breath However a study among Swiss army recruits on Prevalence of halitosis in young male adults found that only frequency of brushing teeth was significantly associated with oral malodor 25 whereas the other studies have reported associations between measures of oral hygiene and oral malodor Miswak a traditional chewing stick is a natural toothbrush made from twigs of the Salvadora Persica tree It is widely used in the Middle East particularly in Saudi Arabia It has an antibacterial effect and is as good as tooth brush in removing dental plaque and reducing gingivitis 25 In our study 55 of those who used toothbrushes and miswak experienced bad breath
