Applicant Organization 201 Cornell Cooperative Extension Albany County CCEAC was established in 1916 as a partnership of Albany County Cornell University and the U S Department of Agriculture which jointly provide a portion of the funding training and administrative support As New York State s land grant university Cornell transfers and applies university based knowledge for practical benefits and contributes to the New York State s economic prosperity CCEAC is a 501 C3 nonprofit organization with an extensive history of providing educational programming and assistance to county residents of all ages CCEAC functions to extend the resources of the land grant institution to address public needs through non formal non credit programs that sustain agriculture and natural resources strengthen youth families and communities and improve the economy Extension programming focuses on major areas such as Agricultural Resources Family and Consumer Resources Energy Conservation Community and Economic Development and 4 H Youth Development The overarching mission Cornell Cooperative Extension puts knowledge to work in pursuit of economic vitality ecological sustainability and social well being We bring local experience and research based solutions together helping New York State families and communities thrive in our rapidly changing world This mission describes our philosophy for delivering educational programs at a grassroots level CCEAC continually focuses on current trends redefining programs and processes to meet the ever changing priorities of the communities that we serve 202 For over 100 years CCEAC has provided urban suburban and rural residents of all ages and socioeconomic status with timely and relevant programming with considerable success in changing participant behaviors that lead to healthier living CCEAC programming is designed to meet local needs of Albany County residents through grassroots community input and is delivered through a wide variety of outreach methods including workshops trainings publications teleconferencing and volunteer opportunities CCEAC has a long standing commitment to the provision of health care promotion education and outreach services to a variety of all age all literacy level audiences from urban settings to rural community outreach centers 203 4 His a youth development program administered by CCE and provides experiences where young people engage in hands on participation in areas such as agriculture health STEM garden based learning and environmental and civic education

Youth are guided by adult mentors to prepare them for leadership roles in their community The 4 H pledge I Pledge My Head To Clearer Thinking My Heart To Greater Loyalty My Hands To Larger Service And My Health To Better Living For My Club My Community My Country And My World philosophy and mission are demonstrated in their pledge CCEAC has many years of experience conducting educational programming with all audiences including limited resource families in all parts of the county urban suburban and rural A variety through a variety of programs funded by the United States Department of Agriculture NYSDOH OTDA DEC EPA as well as private grants and contracts Nutrition Education Programs NYSDOH Healthy Neighborhoods Program we have been able to address preventable health concerns delivered through a wide variety of outreach methods including workshops trainings publications teleconferencing and volunteer opportunities 204 CCEAC utilizes a number of community partnerships to leverage support for our mission and has an excellent reputation with a variety of federal state county and other entities both public and private CCEAC has a long history of working with and supporting limited resource families as they make the transition to self sufficiency 205 Implementation of many of CCEAC s educational programs is possible through partnerships with WIC agencies food pantries community health centers Head Start organizations Albany County Department of Health faith based and community based organizations libraries public housing authorities elementary and secondary schools child care centers and other community based settings 206

A Resource Educator will be responsible for program oversight orientation training and reporting requirements Ms Sara Jones is currently working as a 4 H Urban Educator for Cornell Cooperative Extension of Albany County She recently received her Master of Science in Education in Higher Educational Leadership from The College of Saint Rose where she also received her M S Ed in Literacy Birth to 6th grade and her B S in Childhood Education Special Education Prior to working at Cornell Cooperative Extension she was a coordinator for a grant received by the Department of Education Office of Special Education Programs Email Sj462 cornell edu W 518 765 3546 C 518 207 For over 100 years CCEAC has provided administrative fiscal and project oversight over a variety of grants and contracts from multiple federal state and local agencies All projects were successfully completed reaching deliverables and in some cases exceeding proposed outcome projections Each of these projects required detailed records of all project activities and client interactions and met all reporting requirements including final reporting as requested by the pass through agency Additionally each program received an A133 Single Audit Report conducted by an independent Certified Public Accounting firm Current projects funded through the NYSDOH include Healthy Neighborhoods Program Radon Education Program and Hudson River Fish Advisory
